How can I book my vaccination?

You can order it by calling +420 777 763 155 or by phone at +420 222 094 121 during standard working hours (ideally around 4.00 p.m.) or by e-mail ccm@prophylaxis.cz or using an e-form that you can find on our websites.

What do I need to take along?
For vaccination purposes bring along your health insurance company ID card, certificate of vaccination, date of last tetanus re-vaccination, for children a certificate on poliomyelitis vaccination and necessary cash money or credit card for payment for vaccination according to current price list. We have most of the vaccines on stock.

How to pay for vaccination?

We accept credit cards VISA, MasterCard, Maestro, cards Sphere, Carte and Senior Pas. We also accept vouchers Sodexo and Ticket.
